Please note: this form is not directly attached to financial assistance, but we are able to connect you to programs that may have access to emergency support. To receive the best help, fill out the form as thoroughly as possible.
What to expect after you submit the form:
1) Someone on our team will send you a text with next steps relevant to your prioritized goals as soon as possible (usually in 24-48 hours). If you do not hear from someone, you can send a message through our contact us form!
2) We’ll connect you to available resources / care providers that are specific to you and your goals and lived experience. If you’re already working with someone or tried something already and it didn’t work, you can choose whether or not you want to include that in your “care plan.”
3) A Goal Navigator on our team will follow up with you after making the connection to see if anything changed and if there’s any additional support you might need to access that resource.
Here is everything we consider as a basic need that we hope we can help connect you with:
Childcare / Children's Education: Daycare vouchers, After-school programs, tutoring, etc.
Utility Assistance: LG&E and Water
Eviction Prevention: Legal Support and Rent Assistance
Workforce Development: Finding a job, getting ready for a job, adult education, etc.
Case Management: A coach to help you take next steps toward your goals
Health Care: Insurance, transportation to appointments, access to medication, medical debt/cost, etc.
Food: Applying for food stamps, pantry support, fresh and healthy eating, etc.
Income and Debt Management: Banking, credit repair, financial coaching, etc.
Behavioral Health: Counseling, therapy, support groups, etc.
Recovery Services or Harm Reduction for Substance Use Disorder
Parenting Support & Family Preservation
Intimate Partner Violence Support & Navigation
Navigating Homelessness: Shelters, Outreach team support
Pregnancy Support: Prenatal and postpartum
Foreclosure Prevention
Social Support and Community Involvement
Supplies: Hygiene items, Furniture, clothing, etc
Legal Support: Discrimination and justice work
Finding Housing: Anything from applying for Section 8 to home ownership

We also know that many of these needs are not covered by citywide programs, but we’ll always be honest about what exists and doesn’t and we’ll always fight to expand programming so no one has to be without any of the above. We hope you’ll join us in those efforts.
